Name in the back of the book - what does that look like?

This is the list in the back of Rise.
Not sure why it’s upside down when I post it, it’s not on my computer. Oh well, that gives you a general idea.

I can’t answer this for physical books, but in the Inkshares ebooks I’ve read, there has been a list of names at the back.

Can anyone provide an example of what it looks like to see someone’s name in the back of their book? 

To help explain, I’m referring to when someone purchases enough paperback copies that they get their name included (see copy below):

  • YOUR NAME IN THE BACK OF THE BOOK
  • MULTIPLE SIGNED PAPERBACK COPIES
  • AN EBOOK
  • ACCESS TO DRAFTS AND UPDATES FROM THE AUTHOR
I’m wondering whether every book contains a single list, or a person’s name is printed on an individual unit basis. 

Does this still apply for Quill books?
